Midway
College Faculty Evaluation Form
The Midway College faculty member who is working with the student
teacher will complete this form during an observation of the student
teacher once during the 8-week placement. The faculty member will
complete the form electronically, print out the completed form to
share with the student and use the results as part of the student’s
grade for EDU 498 as well as evidence for Checkpoint 3: Program
Completion.
